Please come 10 minutes early as we will need to fill out a pre-examination form and register patients.When you come to the hospital, you will be asked to show your insurance card and fill out a pre-examination form.We would like to clearly know the patient's symptoms (symptoms that motivate him/her to visit the hospital) and physical conditions (allergies, medications, etc.), so please fill in as much detail as possible.
